CCSA’s Issues of Substance Conference

CCSA Online Conflict of Interest Declaration

As part of their agreement to work with the Canadian Centre on Substance Use and Addiction (CCSA), all partners, sponsors and exhibitors are required to complete CCSA’s Conflict of Interest Declaration. This process has been instituted to protect both CCSA and its partners, and to ensure that our collaborations remain in line with our organizational values and purpose.

Navigating to the links below, please complete the Conflict of Interest Declaration in addition to either the Declaration for Individuals or the Declaration for Organizations, depending on whether you are declaring on behalf of yourself or your organization.

For any questions about filling out the forms or CCSA’s Conflict of Interest Policy: Partners and Sponsors, please email us.
